Welcome new Patients! We hope that the information in this section will be of help to you and assist us in your care. Below we will list some things you should do to prepare for your first visit.
First, we would appreciate it you would complete, print, and bring these forms before your appointment. We also request that you read the privacy notice to learn how we protect your personal health care information.
We also ask that you bring any recent X-ray, MRI, CAT Scan or Bone Scan films including the radiologist report in connection with the problem for which you are being seen.
Also remember to bring your insurance card(s) with you. To see a list of available insurance companies with whom we participate, click here. Although we will submit and help you with any claims, you may still need to make a deductible payment during your office visit
If your injury is related to an auto accident or workers compensation claim, you will need your claim number and the corresponding billing information. Any disability form that we must fill out requires a $15.00 payment.
